Nature of Complaint:

The Physician entered into an Initial Agreement with the Iowa Physician's Health Program (IPHP)and agreed to comply fully with the recommendations including asessement, counseling and monitoring for mental health and substance abuse.

In November 2006, the IPHP asked the Physician to enter into a contract for continued mental health and substance abuse monitoring. The Physician declined to enter into a contract and his not complied with the IPHP recommendation.

Action Taken:

The Board has issued the Physician a Citation and a Warning and issued a Fine of $2500.00.

The Physician is also placed on Probation for five years, to include the following:

1. The Physician shall establish a board monitoring program.
2. The Physician shall not consume alcohol or any unprescribed controlled substances.
3. The Physician shall be subject to drug screening.
4. The Physician shall undergo mental health and substance abuse counseling.
5. The Physician shall obtain a worksite monitor.
6. The Physician shall submit quarterly reports to the Board.
7. The Physician shall appear before the Board as requested.

Nature of Complaint:

The physician has been the subject of disciplinary action in another state.

The physician has been disciplined by the State of Iowa.

Action Taken:

The board has placed the physician's license to practice medicine in the State of Illinois on Probation for a period of five years.
