Nature of Complaint:

The Physician failed to maintain adequate medical records on a patient.

The Physician removed an original office note from the patient's chart that stated the patient was to be seen for a follow-up visit in three months. He replaced the note with one stating follow-up in three weeks and destroyed the original note.

Action Taken:

The Board has issued a Censure and a Reprimand and placed the Physician's license on Conditions for twenty four months. These conditions include a practice monitor and completion of a course i the area of professional ethics.
