The Physician has completed an inpatient treatment program and is seeking reactivation of his license to practice medicine in the State of North Carolina at this time.

Action Taken:

The Board has granted the Physician a license to practice medicine to include the following:

  1. The license is set to expire on 11-23-05.
  2. The Physician shall meet with the Board in September 2005 and at other times at the Board's request.
  3. The Physician shall maintain a contract with the North Carolina Physicians Health Program.
  4. The Physician shall maintain sobriety.
  5. The Physician shall submit to screening as requested by the Board.
  6. The Physician shall attend AA or other sel-fhelp organizations.
  7. The Physician may practice only in a Board approved setting.
  8. The Physician shall provide a copy of this Order to all employers.
