
Nature of Complaint:
The Physician has been charged with unprofessional conduct in his treatment of some female patients.
The Physician has been charged with fondling female patients and was to have a chaperone present at all visits. The Physician was charged with violating this Order.
The Physician is scheduled for a hearing at a later date.
Action Taken:
The Board has ordered the Physician to restrict his practice of medicine to male patients only.
April 5, 2006-Interim Order
The Board has Summarily Suspended the Physician from the practice of medicine, following the results of an evaluation that recommended that he not practice until he receives further treatment.
Nature of Complaint:
The Physician failed to maintain adequate medical records on a patient.
The records failed to contain information that justified the diagnosis, the treatment, warnings and risks and did not provide sufficient information to allow another practioner to assume continuity of the patient's care.
Action Taken:
The Board has issued the Physician a Letter of Reprimand and placed the license on Probation for one year to include Education in the area of recordkeeping and patient management in primary care settings.
Nature of Complaint:
The Physician failed to meet the standard of care in his care and treatment of a patient.
The patient presented to the Physician with a complaint of back pain. At the time of the initial visit, the Physician did not document the severity of the pain or whether or not he performed a back examination. during subsequent visits, the Physicain documented the patient's back pain and referred to it as pain management.
Action Taken:
The Board has issued a Letter of Reprimand.
Nature of Complaint:
The physician admitted to violating a board order by engaging in the practice of medicine, practicing medicine under an assumed name, and writing an undated prescription for Neurontin.
Action Taken:
The physician has Surrendered his license to practice allopathic medicine in the State of Arizona to the board.
Nature of Complaint:
The Physician used a hand-held electronic device to record patient encounters and often copied in the same notes as previous visits with changes only in the date.
The notes made it appear that his long term chronic patients were stable and that nothing changes week to week. This did not reflect the current condition of his patients.
Action Taken:
The Physician voluntarily completed a medical record keeping course and is issued a Decree of Censure and a Civil Penalty of $10,000.00.
