The Board has placed the Physician in Refuse to Renew status.

Nature of Complaint:

The Physician has been the subject of disciplinary action in another state.

The Physician has been disciplined by the State of Ohio.

Action Taken:

The Board has issued a Reprimand.

Nature of Complaint:

The Physician has been diagnosed following a Board ordered evaluation with a personality disorder not otherwise specified.

This disorder renders the Physician incapable of practicing medicine according to standards unless the Physician is closely monitored.

Action Taken:

The Board has placed the Physician on Probation to include the following:

1. The Physician shall submit quarterly reports to the Board.
2. The Physician shall appear before the Board as requested.
3. The Physician shall continue in psychiatric treatment.
4. The Board may request screening on medication levels that the Physician is currently taking.
5. The Physician shall obtain a monitoring physician.
6. The Physician shall provide a copy of this Order to all facilities where the Physician has privileges.

January 12, 2006-Probation Modified

The Board has granted the Physician's request to reduce psychiatric sessions to once a month and Board appearances to once every six months.

The board has approved the physician's request to reduce the number of personal appearances to an annual appearance.
